Job purpose:

The External Case Management Worker is responsible for managing and following up on cases involving persons with disabilities and their peers affected by conflict (especially those in need of rehabilitation and psychosocial support) by providing them with case management services and internal and external referrals under the direct supervision of the Case Manager.

Duties, objectives, and competencies:

  • The Case Worker will operate in the Idleb Kafr Takharim area, with regular field visits to local actors in the region, under the direct responsibility of the Case Manager, as follows:
  • Participate in and support the identification of cases of children with disabilities and their peers who are most at risk.
  • Conduct risk and needs assessments, develop care and safety plans, organize case conferences, and make referrals.
  • Refer children with disabilities to appropriate support.
  • Contribute to the development of targeted and proposed activities for case management.
  • Implement individual case management plans as part of the case management program, coordinating directly with the Case Manager for each case.
  • Identify cases in need of direct individual support and serve them according to the severity of the case.
  • Attend regular meetings with the Case Manager and provide updates on work progress.
  • Ensure proper and accurate documentation of all case files in accordance with the information management system of the protection program.
  • Respond to and handle high-risk cases directly, in coordination with the Case Manager.
  • Network with psychosocial support and special education teams, and facilitate referrals to formal and non-formal education under the supervision of the Case Manager, while providing necessary support for the proper integration of children with disabilities into formal and non-formal education settings with their peers.
  • In collaboration and coordination with the direct supervisor (Case Manager), work on creating a service map to ensure cases are referred and supported in a timely and appropriate manner.
  • Perform all other protection-related activities in coordination with the direct supervisor.
  • Write activity and weekly reports.
  • Perform any other tasks requested by the direct supervisor.

  • Attending periodic meetings with the case manager and giving updates on work progress.
  • Ensure correct and accurate documentation of all case files in accordance with the protection program’s information management system.
  • Respond and deal with severe risk situations directly in coordination with the case manager.
  • Networking with psychological support and special education teams and referrals to formal and informal education under the supervision of the case manager, in addition to providing the necessary support for the proper implementation of the integration process into formal and informal education classes for children with disabilities with their peers.
  • In cooperation and coordination with the direct manager (case manager), work on creating a service map to ensure that cases are referred and supported in the appropriate manner and at the appropriate time.
  • Carrying out all other protection-related activities in coordination with the direct manager.
  • Writing activity reports and weekly reports.
  • Any other tasks requested by the direct manager.

Requirements: Essential

  • University degree in Special Education or Social Work.
  • Beginner level in English language skills.
  • Good experience working with persons with disabilities
  • At least one year of experience with international or local NGOs.

  • At least one year of previous experience in social work and case management.
  • Experience working with case files and data management.
  • Knowledge of effective humanitarian and social agencies in the region.
  • High communication skills.
  • Ability to work within a team.
  • Good communication, strong interpersonal skills and the ability to work in different cultural environments.
  • Self-motivated and able to handle work pressure.
